Exosomes 101: Are These Pricey Products Worth It?

Exosomes are showing up everywhere right now, in serums, in med spa menus, and in injectable treatments, and I get asked about them almost every day. I have been measured in my enthusiasm, because most of what is being sold is running ahead of the science. In this episode I sat down with Alisa Lask, CEO of Rion Aesthetics and the lead behind Plated Skin Science, someone who comes from pharma and biologics and has actually invested in human research. Here’s what you need to know before you spend a dollar on anything labeled ‘exosomes.’

The Bottom Line

Exosomes are a genuinely exciting area of science, but most exosome skincare products are running ahead of the evidence. The source matters far more than the word on the label, no exosome product is FDA-approved for aesthetic use, and topical penetration is a real open question. A small number of products backed by peer-reviewed human research stand apart from the rest, and that distinction is what should guide your money.

What Are Exosomes, Really?

Exosomes are tiny vesicles, meaning membrane-bound sacs, that your cells produce naturally throughout the body. They carry cargo like growth factors, proteins, lipids, and microRNA, and they work as a form of cell-to-cell communication. An exosome binds to receptors on another cell and influences how that cell behaves.

In the skin specifically, exosomes play a role in cell maturation, cell differentiation, blood vessel growth, and the regulation of inflammation. That last part is a big reason the research world is interested, because an ingredient that can influence inflammation and immune response becomes an attractive candidate for conditions like acne, acne scarring, rosacea, and eczema, and for wound healing and recovery after procedures.

So the underlying biology is real and it is interesting. The gap between that biology and the serum on a shelf is where I want you to slow down.

Why Is Everyone Talking About Exosomes?

The excitement is not made up. Early work suggests exosomes may improve results when added to procedures like microneedling and laser, compared with the procedure alone, and there is active research in hair restoration and in cellular aging. The problem is that most of the compelling data still sits at the preclinical level, which means cells in a dish and animal models rather than human studies.

Preclinical research is meant to guide human research, not replace it. In medicine, things that look extraordinary in a dish frequently do not translate to real skin on a real person, and that is the entire reason we run clinical trials. The human data that does exist for exosomes is small, early, and limited. Promising is not the same as proven, and a lot of this category is being sold as if that translation is already settled. It is not.

Why the Source of Your Exosome Matters More Than the Label

Here is the single most useful thing to take away. Not all exosomes are the same, and the word exosome on a box tells you almost nothing.

Exosomes can be derived from different sources, including platelets, mesenchymal stem cells, and plants. These are not interchangeable. An exosome derived from a plant is a fundamentally different particle than one derived from human platelets, and it may have little relevance to human skin biology at all. Human-derived exosomes however are more biologically relevant to skin and human issues. 

The category is also loosely regulated, so a brand can put exosomes on a label without much rigor behind it. When production is not tightly controlled, the integrity of the exosomes themselves is in question, and you can end up paying a premium for particles that are damaged, sparse, or not what the label claims.

Can Exosomes Even Penetrate Your Skin?

This is one I think about a lot, because it does not matter how good an ingredient is if it cannot reach where it needs to go. Exosomes are nanoscale particles, and the outermost layer of your skin, the stratum corneum, is a genuine barrier designed to keep things out.

The evidence here should make you cautious about bold topical claims. In one study of human skin, fluorescently tagged exosomes applied to the surface stayed almost entirely within the stratum corneum, with less than one percent of the material making it through over 24 hours. Microneedling and laser can create channels that help delivery, which is exactly why exosomes are so often paired with those procedures. Whether microneedling helps the exosomes do their job or stresses them in the process is a fair question, and one worth asking your doctor directly. 

Formulation and storage matter here too. These are delicate biologic particles, and they can be destabilized by other ingredients, by temperature, and by improper storage, which is part of why a shelf-stable, well-formulated product is not a small engineering feat.

What Does the Research Actually Show?

At the category level, the honest summary is early and limited. A systematic review of exosomes for hair loss identified only a handful of clinical studies, including a small number of randomized controlled trials, and while some showed improvements in hair density with a reasonable safety signal, the authors were clear that human data remains limited. That is the pattern across most exosome applications right now.

Are Exosome Products FDA-Approved?

No. As of 2026, no topical or injectable exosome product is FDA-approved for aesthetic use. The FDA regards exosome products intended to treat a condition as drugs and biologics that require approval before they can be marketed that way, and the agency has issued warning letters to companies selling them. Products currently reach the market as cosmetics or as research-use-only formulations, not as approved treatments.

This matters most for injectables. In my view, injecting exosomes is well ahead of the evidence right now, and the lack of regulatory oversight means you cannot always be confident about what is in the vial or how it was handled. Topical, well-studied products are a different and more reasonable conversation than injecting an unapproved biologic into your face.

Who Should Be Careful With Exosomes?

A few groups should approach this category with more caution and a real conversation with their dermatologist. If you have a history of precancerous lesions or skin cancer, remember that exosomes influence cell behavior and growth, and the long-term data simply is not there yet to say how that plays out. The same caution applies if you are immunocompromised or have a significantly compromised skin barrier. This is not fearmongering, it is an honest acknowledgment that we do not yet have the studies to answer these questions, and responsible use means factoring that in.

So Why Do I Use an Exosome Product Myself?

I do use an exosome product daily that has peer-reviewed human research, a platelet-derived source that is relevant to skin, and a formulation built for stability rather than hype. I genuinely believe that exosomes harness a real potential for growth in medicine and skin health in the future, but it’s important to stick with products that have data, are scientifically sound and are sourced appropriately.

MYTH: All exosomes are basically the same. REALITY: Exosomes derived from platelets, stem cells, and plants are fundamentally different particles with different relevance to human skin. The source matters far more than the word exosome on the label.

MYTH: Exosome serums penetrate deep into your skin on their own. REALITY: Exosomes are nanoscale particles, and the stratum corneum is a strong barrier. In one human study, less than one percent of topically applied exosomes passed through over 24 hours. Delivery, formulation, and sometimes a procedure like microneedling all affect whether they reach living skin.

MYTH: If it is being sold and it is expensive, it must be proven. REALITY: No exosome product is FDA-approved for aesthetic use, and most have no peer-reviewed human data behind them. Price is not evidence.

MYTH: Injectable exosomes are the cutting edge, so they must be worth it. REALITY: Injecting exosomes runs well ahead of the current evidence and the regulatory framework. This is the application to be most cautious about, not the most eager for.

Is it safe to have someone else’s platelets in your skincare? Donor-derived platelet exosome products rely on screened donor material, similar in principle to other donor-derived biologics, and manufacturers apply screening and processing controls. That said, exosomes are a newer and loosely regulated category, so it is fair to ask a provider about sourcing, screening, and manufacturing standards.

Should I microneedle exosomes into my skin? Microneedling can help exosomes get past the stratum corneum, which is why the two are often paired, but it should be done by a qualified provider using a product appropriate for that use. Applying exosomes into open microchannels also raises the importance of product sterility and quality, so this is not a casual at-home step. Discuss it with a board-certified dermatologist rather than improvising.

Where This Leaves You

Exosomes are fascinating science with real long-term potential, especially for inflammatory conditions, healing, and as add-ons to procedures. They are also early, loosely regulated, and surrounded by marketing that has sprinted past the data. You do not need to avoid the category entirely, and you do not need to buy into the hype. You need to ask better questions. What is the source, is there peer-reviewed human research, how is it delivered and stored, and is anyone claiming FDA approval that does not exist.

If a product cannot answer those questions, keep your money. If it can, you are making an informed decision rather than a hopeful one, and that is the whole goal.
