If you have a half-used tube of tretinoin sitting in the back of your bathroom drawer, you are in good company. I see patients give up on this ingredient every week, almost always because of one of the same five mistakes. Most people walk away right before tretinoin starts to deliver the clearer, smoother skin they came in for. Every one of these missteps is small and fixable, and I have made several myself.
What Tretinoin Is and Why It Is Worth Using Correctly
Tretinoin is the prescription-strength form of vitamin A, also known as all-trans retinoic acid. Unlike over-the-counter retinols that have to convert in the skin before they work, tretinoin acts directly to stimulate collagen, speed up cell turnover, fade pigment, smooth texture, and treat both acne and signs of aging (Mukherjee S, et al. Retinoids in the treatment of skin aging. Clin Interv Aging. 2006). It has been a dermatology mainstay for over forty years, and most dermatologists I know use some form of it themselves. Real results take three to six months of consistent use, which is exactly why getting these five details right matters.
Mistake 1: Quitting During the Purge
If your skin breaks out, flakes, or looks worse two to six weeks in, that is not tretinoin failing. It is doing what it should, which is push clogged pores to the surface faster than they would arrive on their own. This is the purge, and it is temporary.
Retinoid dermatitis is different. Stinging, burning, persistent redness, and tightness in areas where you do not normally break out are signs of true irritation. If you cannot tell the difference, scale back frequency and focus on barrier repair.
Mistake 2: Using Too Much Product
More tretinoin does not mean faster results. It just means more irritation. A pea-sized amount is enough for the entire face. Dot it on the forehead, both cheeks, and the chin, and gently spread it out. That is it.
Mistake 3: Using It Too Often, Too Soon
Going nightly from day one will almost always trigger irritation, and irritation is the number one reason patients abandon tretinoin before it has a chance to work.
A more realistic starting cadence looks like this:
- Weeks 1 to 2: two nights per week
- Weeks 3 to 4: three nights per week
- Weeks 5 and beyond: increase as tolerated, often working up to nightly
If your skin feels reactive, drop back a step and stay there until it calms.
Mistake 4: Treating It Like a Spot Treatment
Tretinoin is not a pimple cream. Used only on one spot, it tends to over-irritate that small patch of skin without giving you the broader benefits, like improved tone, smoother texture, softened fine lines, and long-term collagen support. Apply it across the full face as a thin, even layer.
Mistake 5: Layering It With the Wrong Actives
Layering tretinoin with too many other actives is one of the most common ways patients end up with a compromised barrier. The combinations causing the most trouble include:
- Tretinoin plus benzoyl peroxide on the same night, which can inactivate the tretinoin and increase irritation
- Tretinoin plus AHAs or BHAs in the same routine
- Tretinoin plus a strong vitamin C in the same evening
A gentle cleanser, a basic moisturizer, tretinoin at night, and broad-spectrum sunscreen by day is a complete regimen for most people.
The Sandwich Method: Your Sensitive-Skin Workaround
If your skin runs sensitive or you are easing into tretinoin for the first time, try the sandwich method. Apply a thin layer of moisturizer to clean, dry skin, wait a few minutes, apply your pea-sized amount of tretinoin, and follow with another layer of moisturizer on top. This buffers the active without blocking its benefits, and it has helped many of my patients finally tolerate tretinoin long enough to see real results.
Tretinoin works. The problem is almost never the molecule itself, and almost always how it is being used. Use a pea-sized amount, build up frequency slowly, apply it across the whole face, simplify your routine, and reach for the sandwich method when your skin needs extra support.
