You’ve tried the serums. You’ve patted in the eye creams. You’ve even gone to bed with cold spoons on your face. And yet those under-eye concerns are still staring back at you every morning.
Here’s the truth: you might be treating the wrong problem entirely.
Watch the full episode on The Skin Real for the complete breakdown and expert guidance.
The Biggest Mistake People Make With Under-Eye Care
When it comes to under-eye concerns, most people treat symptoms without understanding the type of issue they’re dealing with. There isn’t just one kind of “under-eye problem.” There are four distinct types, and each one requires a completely different approach.
Mixing them up doesn’t just waste your money. It can actually make things worse.
The 4 Types of Under-Eye Concerns (And How to Tell Them Apart)
1. Puffiness
This is the classic “I woke up looking swollen” concern. True puffiness is usually caused by fluid retention. Think too much sodium, alcohol, allergies, or a poor night’s sleep. It tends to fluctuate day to day.
What actually works: Caffeine-based eye products can help temporarily constrict blood vessels and reduce swelling. Cold therapy, like a chilled eye mask or a cold compress, is also an effective and low-cost option.
What to avoid: Surprisingly, Botox around the eye area can actually backfire for puffiness sufferers. It can relax the muscles that normally help drain fluid, making swelling worse over time.
2. Fat Pad Herniation
This one looks like puffiness but doesn’t go away, even with sleep, water, and a clean diet. That’s because it’s a structural issue. The fat pads around your eyes have shifted forward and are now visible beneath the skin.
What actually works: This is one of the few under-eye issues that requires a surgical solution. A lower blepharoplasty, performed by a board-certified surgeon, is the gold standard treatment for fat pad herniation.
What to avoid: Dermal filler is not the answer here. In fact, adding volume to an already herniated fat pad can make the bulging appear more prominent, not less.
3. Volume Loss and Hollowing
This is the opposite of puffiness. As we age, the fat pads under our eyes can actually shrink, creating a hollow, sunken, or shadowy look. You may hear this referred to as the tear trough area.
What actually works: Tear trough filler, when placed carefully by an experienced injector, can restore volume and soften the hollow look. Biostimulants like Sculptra or Radiesse are another option that encourages your skin to produce its own collagen over time.
What to avoid: Don’t confuse this with puffiness. Adding filler to an eye that looks puffy due to fat pad herniation will only worsen the appearance.
4. Dark Circles
Dark circles can come from two completely different sources, and the treatment depends entirely on which type you have.
- Pigment-based dark circles look brown or grayish and are more common in deeper skin tones. These respond well to topical treatments like hydroquinone, as well as certain laser therapies that target melanin.
- Vascular dark circles look purple or bluish and are caused by blood vessels showing through thin under-eye skin. These are best treated with vascular lasers that target the blood vessels directly.
What to avoid: Spending money on generic “dark circle” creams won’t do much if you haven’t identified which type you’re dealing with. Most over-the-counter products don’t differentiate, but your treatment plan absolutely should.
How to Figure Out What You’re Dealing With
Still not sure? Here’s a quick self-check:
- Does it go away after a good night’s sleep? Likely puffiness.
- Does it stay, no matter what? Could be fat pad herniation or hollowing.
- Is it more shadow-like and sunken? Volume loss.
- Does it have a brown, purple, or blue tint? Dark circles, either pigment or vascular.
When in doubt, a consultation with a board-certified dermatologist or plastic surgeon can give you a clear diagnosis and a personalized treatment roadmap.
The Bottom Line
Your under-eye area deserves more than a one-size-fits-all approach. Before you invest in another serum or book a filler appointment, take a step back and ask yourself what is actually going on.
Treating the right problem with the right solution is the only thing that truly works.
